Both the Risk Waiver form and Confidential Medical History form must be completed prior to participating in equestrian programs or sessions for the first time and emailed to nina@evolvewithhorsecoaching.com.au. 

Please note: If medical conditions change participants will need to complete the confidential medical form - with updated information and forward to above email address.
